EMS Cost Recovery

EMS Cost Recovery is the process of obtaining reimbursement for expenses of providing emergency ambulance transportation. Medicare, Medicaid and most health insurance policies already allow for this reimbursement. When a citizen calls 911, an ambulance is dispatched immediately. Patients will be treated, and if necessary, transported to a hospital emergency department. Patients will be asked to sign a Patient Care Report for billing purposes. If patients are unable to sign, a claim form will be forwarded to their health insurance provider.

About Fare Care

Fare Care is a subscription program to help citizens alleviate any out-of-pocketFare Care expenses, such as health insurance co-payments and deductibles, when emergency ambulance transportation is needed. Individuals may be billed for emergency ambulance transportation under the city’s Cost Recovery Program. Fare Care is your protection against rising health care costs. Your Fare Care subscription will also pay emergency ambulance transportation expenses for a subscriber who does not have health insurance.

Subscriptions

Subscribe to FARE CARE by enrolling for only $29 a year (single coverage), or $59 a year (2 or more family members). Employees who work in, but do not reside in Colonial Heights, are also eligible for enrollment. Your FARE CARE subscription will be effective after your payment and signed enrollment form are received. Enrollment is non-refundable and non-transferable. Late subscriptions to the program will be accepted, but subscriber fees are not pro-rated.

Program Terms

For an annual cost of $29 for single coverage, or $59 for two or more people, your FARE CARE subscription defrays out-of-pocket expenses for the uninsured or any underinsured portion of charges for an emergency ambulance transportation that ends at a hospital. A FARE CARE subscription covers individuals who reside at the address listed on the enrollment form. The enrollment form must be completed and returned with payment no later than August 31 of each year. You will not be eligible to subscribe at the time our services are rendered. Fare Care Subscriptions are valid September 1 through August 31 of each year and are non-refundable and non-transferable.

Download and complete the Fare Care Application. Mail the application and your payment (made payable to City of Colonial Heights) to Colonial Heights Fire Administration, P.O.
