How do I file a claim with the City's insurance carrier?
Claim forms are available in the City Attorney's office at:
201 James Avenue
Colonial Heights, VA 23834
Upon receipt of the completed claim form and estimate for repairs, the city attorney will forward the claim to the risk manager / insurance carrier for a determination of the claim.
